Radiology collaborative

The HVPAA Radiology Collaborative is co-directed by Pamela Johnson, MD from Johns Hopkins Hospital and Yoshimi Anzai, MD, MPH from University of Utah. With the approval of their department chair, 30 academic medical centers (listed below) joined the radiology collaborative and are participating on multicenter performance improvement initiatives designed to improve the value of care we deliver.

Our mission:

  • Increase appropriate use of imaging exams by decreasing low value tests and increasing high value exams, through performance improvement initiatives and education
  • Deliver high value interpretations that optimize use of downstream tests and treatments
  • Protect patients from unnecessary surgery, emergency department visits and avoidable hospitalizations

Our initiatives:

  1. Consensus standardized management recommendations based on ACR AC®, ACR incidental finding white papers and other society evidence-based guidelines.
    • Phase I: lung nodule, thyroid nodule, AAA
    • Phase II: adrenal nodule, ovarian cyst, small renal mass
    • Phase III: pancreatic cyst, renal cyst
  2. Harmonizing abdominal CT protocols to reduce unnecessary multiphase imaging, which delivers a higher radiation dose and a higher cost for the patient, as detailed in the new ACR Choosing Wisely® recommendations
  3. Developing standardized reporting to better guide management decision making: leading PE-RADS with the American College of Radiology
  4. Pooling data to establish benchmarks for emergency department imaging positive exam rates before and after performance improvement interventions
  5. Rad/EMed/ID: Reduce imaging-driven antibiotic overuse in nonbacterial lower respiratory infection and noninfectious bacteriuria/pyuria (pursuing AHRQ funding with ACR CRI)
  6. Design, develop and test measures of radiology value related to optimizing patient management

Our educational program:

A leading principle of high-value practice is avoidance of unnecessary tests and treatments in accordance with established clinical practice guidelines. To reinforce this practice, we designed an in person CME/MOC educational program (Order Wisely®) that reviews appropriate use of imaging exams, lab tests, medications, transfusions and procedures. An understanding of the evidence behind appropriate ordering and prescribing is important to change long-standing practice habits and overcome concerns about missed diagnoses. Included are presentations on best imaging exam selection for the 8 CMS Priority Clinical areas, to ensure that providers are prepared for the CMS Appropriate Use Program, which will include evaluation of their ordering appropriateness beginning in January 2020.
